Why you need it on the job
Non-licensed does not mean unregulated. The plan of work has to exist before the task starts, and it is what proves the controls, the waste route and the decontamination were decided in advance rather than improvised.

The equipment and controls this template asks you to record are governed by published standards that define what each item must withstand and how it is tested. These are the ones that apply most directly:

What separates non-licensed from licensed work?
The material type, its condition and the fibre release expected. Anything above the threshold, or any work on sprayed coating, lagging or insulating board, needs a licensed contractor.
Is notification always needed?
Notifiable non-licensed work must be notified before it starts and requires medical records and health surveillance. The checklist covers which category the task falls into.
