The most powerful safety lessons come from people who were there. If you have witnessed a near miss, an equipment failure or an incident that taught you something, sharing it can help others avoid the same harm. This page explains how to submit your story.
What makes a useful story
Helpful submissions describe what the task was, what went wrong or nearly went wrong, and what you learned. You do not need polished writing; a few honest paragraphs about the situation, the outcome and what you would do differently are enough for us to work with.
Your privacy matters
Stories are published anonymously by default. We remove company names, personal names and any detail that could identify a specific person or site unless you specifically ask us to keep them. The goal is to share the lesson, not to point fingers.
How to send it in
Once our submission form is live you will be able to send your story directly from this page. In the meantime, if you have something to share, reach out through the site and a member of the team will help you write it up safely and clearly.
