Why you need it on the job
Once you employ five or more people the written policy stops being optional, and clients ask for it at pre-qualification whatever your size. The three-part structure is what auditors expect, so producing it in that shape avoids the document being sent back.

Where this template names protective equipment or a control measure, the equipment itself is governed by a published standard that defines what it must withstand and how it is tested. What are the three parts?
A statement of intent signed at the top of the business, a section setting out who is responsible for what, and the arrangements describing how safety is actually delivered.
How often should it be reviewed?
At least annually, and immediately after any significant change in what the business does, who runs it or how the work is organised.
