What makes a risk assessment “defensible”?
One that shows hazards were genuinely identified, likelihood and severity were judged honestly rather than minimised, the chosen controls are proportionate to the risk, and the reasoning is recorded clearly enough that another person could follow the same logic.
Should every conceivable hazard be included?
The assessment should cover hazards that are reasonably foreseeable for the actual task and environment, not an exhaustive list of theoretical risks with no realistic bearing on the work being done.
Why does the recorded reasoning matter as much as the conclusion?
Because a risk assessment is judged on whether a reasoned judgement was demonstrably made, not just on the final control chosen – the written reasoning is the evidence that the process was genuine.
How often should a risk assessment be reviewed?
Whenever the task, equipment, environment or personnel involved changes materially, and otherwise on a periodic schedule appropriate to how quickly conditions on that job actually change.
