The Live Panel That Should Have Been Dead

An electrician opened a distribution panel to trace a fault, believing a colleague had isolated it. It was still live. A dropped tool bridged two terminals and produced an arc flash.

The isolation had been assumed, not proven. There was no lock on the isolator and no test to confirm the panel was dead before work began. The arc caused burns and destroyed the panel; the worker was fortunate the injuries were survivable.

The lesson

Safe isolation means locking off, tagging, and then testing to prove dead every time, with no exceptions. A lockout-tagout procedure and insulated tools rated to the right standard turn a fatal assumption into a verified fact.
